Better pictures would help. But I'd chop off everything above the first branch and make the tree out of that.

the first branch is from the nebari and isn't even on the trunk.

Get it healthy, cut it down to half that height if not more. Then once it pops its new buds down the trunk, cut it back above a new branch at the height that Paul suggested, and regrow it in a natural olive shape. (height where the current branch appears to come off the trunk that is.)

Before cutting the trunk back I'd remove the suckers and clean up the branches so you can see what you have. Also remove any dead bark as you might have some nice deadwood features hiding in there.
